GUIDELINES ON THE SUBMISSION OF SURVEY RETURNS AT DENR-NCR
1. The Geodetic Engineer (GE) is requested to submit via email (This email address is being protected from spambots. You need JavaScript enabled to view it.) the following using his/her official and registered email address:
a. Digital Land Survey Data (DLSD)
b. Survey plan (LMB Form No. GSD-C-1) printed by LMB authorized printing office
c. Completely filled out Transmittal of Survey Returns (LMB Form No. GSD-E-4)
d. Certified true copy of Title, Original Survey Authority/Survey Order, whichever is applicable
e. Completely filled out Field Notes Cover
f. Other pertinent documents (if applicable only), e.g. Deed of Sale, Extra-Judicial Settlement, Special Power of Attorney or Secretary’s Certificate, other Certification, LRA Reference Plan, etc.
2. Once the online preliminary examination is completed, the GE shall be informed via email that his/her Complete Survey Returns (CSR) is ready for submission (or otherwise, informed of the lacking requirements). Likewise, the computed verification/TD fee shall also be provided.
3. The GE or his authorized representative shall go to DENR-NCR office to request the Order of Payment, pay the verification/TD fee at our Cashier and submit the CSR.
